2025-26 M-Pact Baseball Liability Waiver

In consideration of my child’s participation in activities, practices, games, training, or events sponsored or conducted by M-Pact Baseball and/or Magic Softball (collectively “M-Pact”), whether held at the M-Pact indoor facility, outdoor grounds, or any other location: I, the undersigned parent/guardian, acknowledge and agree as follows: Assumption of Risk I understand that participation in baseball/softball and related activities involves inherent risks, including but not limited to falls, collisions, injuries from equipment, weather conditions, transportation, and other players. I voluntarily assume all such risks and hazards incidental to my child’s participation. Release of Liability I do hereby waive, release, absolve, indemnify, and agree to hold harmless M-Pact Baseball, their directors, officers, coaches, staff, sponsors, volunteers, agents, participants, and anyone transporting my child to and from team activities, from any and all claims, demands, causes o
